Job summary

Optum Home & Community Care, part of UnitedHealth Group, is seeking an Advanced Practice Clinician (NP/PA) to join the House Calls team in Snohomish County, WA. You will perform in-home health assessments for Medicare Advantage and other plan members, educating patients and coordinating with PCPs to close care gaps.

You will conduct exams, screenings and health education, with no on-call, weekends or holidays, and flexible hours between 8 a.m. and 8 p.m.

Qualifications

• Active licensure in Washington or ability to obtain by start date
• Current driver’s license and reliable transportation for home visits
• 1+ years of clinical experience; home health or geriatric care preferred
• Proficiency with EMR and telehealth/technology
• Ability to navigate varied home environments during assessments

Responsibilities

• Conduct comprehensive in-home assessments including medical history, meds, vitals and exams
• Perform screenings and point-of-care testing as appropriate
• Document diagnoses for care management and treatment planning
• Communicate findings to members’ PCPs to close care gaps
• Identify urgent or emergent situations and intervene appropriately
• Educate members on disease processes, medications and adherence
• Address social determinants of health and provide referrals as needed
• Flexible scheduling between 8 a.m. and 8 p.m. across a 7-day week; 24–40 hours per week; no on-call
