Job summary

UnitedHealth Group seeks an Advanced Practice Clinician (NP or PA) to join the House Calls team conducting in-home assessments in Snohomish County, WA. You will evaluate health status, perform screenings and coordinate with PCPs to close care gaps.

The role offers flexible scheduling (8am–8pm), 24–40 hours weekly, with no on-call or weekend shifts. Requires WA licensure and NP/PA certification; EMR proficiency is valued, and multilingual candidates are welcome.

Qualifications

• For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PAs: Current NCCPA certification and state licensure
• Active, unrestricted licensure in Washington, or ability to obtain by start date, without current disciplinary actions or practice limitations
• Active and unrestricted driver's license
• Access to reliable transportation to complete home visits
• Ability to navigate varied home environments and sit, stand and kneel as needed to perform assessments
• If working in a state that allows NPs/PAs to practice independently without a Collaborative Agreement, you must obtain approval from your licensing board, if required. New hires who are eligible but have not applied yet must do so within 1 month of starting. If you are not eligible at the time of hire, you must begin working toward eligibility within 1 month and apply for approval within 3 months of becoming eligible

Responsibilities

• Conduct comprehensive in-home assessments, including past medical history review, medication reconciliation, vital signs and physical exams
• Perform evidence-based screenings and point-of-care testing, as appropriate
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to members' PCPs to address gaps in care
• Recognize urgent and emergent situations and intervene appropriately
• Educate members on disease processes, medications and compliance
• Address social determinants of health and provide referrals as needed
• Flexible scheduling, with providers able to flex their time between 8 a.m. and 8 p.m. over the 7-day work week
• Flexible provider schedules available between 24 and 40 hours per week
• No on-call, weekends or holidays required
