Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to conduct in-home health evaluations. You’ll visit members’ homes for about 45 minutes, review medications and medical history, perform a physical evaluation, and optionally arrange additional tests as ordered by the health plan.

You’ll travel locally and within licensed states, work part-time with an hourly base pay and incentive per visit, and access comprehensive benefits for eligible hours.

Qualifications

• Active, unrestricted license in the coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Conduct in-home health evaluations for plan members.
• Travel locally and within licensed states as needed.
• Provide Signify Health services and related evaluations as indicated.
