Job summary

Signify Health, part of CVS Health, seeks a Per Diem Clinician (NP or PA) to deliver in-home health evaluations, visiting members at their homes and helping bridge gaps in care. You will use an iPad and clinical workflow to review medications and medical history, perform a physical evaluation, and, if ordered by the plan, conduct additional diagnostic tests.

The role is part-time with a typical weekly 12 hours and travel within licensed states.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
• Able to cover Boulder/ Grand County (primary locations).

Responsibilities

• Provide in-home health evaluations for health plan members.
• Travel locally within licensed states to patient homes.
• Conduct virtual visits as needed.
• Use iPad and Signify Health clinical workflow during evaluations.
• Provide Signify Health services (Diagnostic & Preventive).
