Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to provide in‑home health evaluations, visiting members in the comfort of their own homes. You will conduct health assessments, review medications and medical histories, and perform basic physical evaluations using an iPad and a clinical workflow.

This part‑time role offers flexible travel to licensed states, virtual visits as needed, and the chance to bridge gaps in care while delivering personalized health

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations.
• Travel locally within licensed states as needed.
• Conduct virtual visits as needed.
• Use Signify Health services and iPad workflow during evaluations.
