Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to provide in-home health evaluations for members in their homes. You’ll review medications, medical history and perform a physical evaluation to help bridge gaps in care.

The role is part-time with local travel, requires active licenses and board certification, and may include virtual visits. You’ll join a national network focused on improving health outcomes.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• KENNEWICK ADAMS Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations to health plan members in their homes.
• Engage with members, bridging gaps to ensure appropriate care.
• Utilize iPad and clinical workflow; conduct virtual visits as needed.
