Job summary

Signify Health, part of CVS Health, seeks a Per Diem Clinician (NP/PA) to perform in-home health evaluations. Travel locally to member homes across licensed states, with visits lasting about 45 minutes, including history review and physical exam.

You’ll work via iPad, delivering personalized care without prescribing or altering treatment plans. This part-time in-home role offers hourly pay with incentives, malpractice insurance, and supplied materials.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.
• Ability to cover Sterling and surrounding area.

Responsibilities

• Perform in-home health evaluations for patients in their residences.
• Review medication history and assess health status during visits.
• Use Signify Health workflow via iPad to document findings.
• Coordinate as needed with health plans for diagnostic tests and referrals.
• Travel locally and within licensed states as required; conduct virtual visits when needed.
• Provide Diagnostic & Preventive Services as indicated by the health plan.
