Job summary

Signify Health, part of CVS Health®, is hiring a Per Diem Clinician (NP/PA) to deliver in-home health evaluations. You will visit patients at their homes, collect medical history, perform physical assessments, and coordinate with health plans.

The role supports flexible scheduling and requires travel to local sites and some virtual visits. The position offers an hourly base rate with incentives, malpractice coverage, supplies, and benefits for part-time staff.

Qualifications

• Active, unrestricted NP/PA licenses in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Visit patients in their homes for health evaluations.
• Travel locally to assigned states as needed.
• Perform in-home health evaluations using iPad and clinical workflow.
• Provide Signify Health services as indicated, including diagnostic & preventive care.
• Conduct virtual visits when required.
