Qualifications

• Master’s Degree or equivalent experience.
• Current NP licensure in state of practice.
• AANP, AACN, or ANCC board certification in a relevant specialty.
• Current BLS, ACLS, or CPR certification.
• At least 1 year of NP experience.
• Ability to travel extensively (80-90%) including overnight.

Responsibilities

• Conduct comprehensive health assessments including medical history and risk factors.
• Administer necessary point of care and lab-based testing.
• Collaborate with healthcare teams for care gap closure.
• Provide services at members’ homes or skilled nursing facilities.
