Job summary

Optum Home & Community Care is seeking an HouseCalls Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. The role focuses on education, gap closure, and coordination with primary care providers.

Flexible scheduling supports work-life balance, with no on-call, weekends, or holidays. This part-time position (20 hours/week) offers meaningful patient interaction in a field-based setting, with compensation aligned to experience and practice

Qualifications

• Must hold NP or PA credential with active licensure in the practicing state.
• Experience in home health or Medicare care settings preferred.
• Ability to navigate varied home environments during visits.

Responsibilities

• Conduct in-home comprehensive health assessments for Medicare Advantage members.
• Document diagnoses for care management and care planning.
• Educate patients on diseases, medications, and preventive care.
• Coordinate with PCPs to close gaps in care and refer to services as needed.
