Job summary

UnitedHealth Group is seeking a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients' homes as prescribed by physicians, document visits, and coordinate other care activities within the service area.

Under supervision, you will ensure high-quality care, follow infection control procedures, maintain accurate electronic medical records, and collaborate with families, physicians, and the health care team to support positive outcomes.

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver's license and vehicle access or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws and plan of care, coordinating with the health care team.
• Documents patient visits thoroughly in the electronic medical record and uses the agency's visit verification program.
• Participates in care coordination, reports patient needs and changes in condition, and communicates with patient, family, physicians, and case coordinators.
• Ability to work in any home setting regardless of age, race, creed, color, sex, disability, or financial condition.
