Job summary

UnitedHealth Group is seeking a Licensed Practical Nurse (LPN) for Home Health to deliver high-quality patient care in clients' homes, coordinating with the health care team to implement the plan of care and ensure infection control procedures.

Responsibilities include documenting visits in the electronic medical record, participating in care coordination, and maintaining clear communication with patients, families, physicians and case managers while adapting to diverse home care situations.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance or access to transportation.
• Ability to work in diverse home-care settings.

Responsibilities

• Provide high-quality clinical services within scope of practice in patients' homes.
• Document visits thoroughly in the EMR per regulatory requirements.
• Participate in care coordination and communicate changes with the care team.
