Job summary

UnitedHealth Group is seeking a Home and Community Based Services Licensed Practical Nurse to deliver direct nursing care at patients' homes under physician orders. You will document visits, support care coordination, and coordinate with the health care team to ensure high-quality patient outcomes.

Under supervision of a Registered Nurse or supervisor, you will operate within state regulations and infection control procedures while providing compassionate care to acutely or chronically ill

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current CPR certification or ability to complete within 90 days of hire.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice and plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and communicates updates to patients, families and the health care team.
