Job summary

UnitedHealth Group is seeking a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients’ homes as prescribed by the physician and to record progress notes. You will coordinate other patient care activities to ensure high-quality care within the assigned geographic service area, under supervision.

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• Ability to work flexible hours and manage multiple tasks simultaneously.
• Able to work independently and collaborate with the health care team.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws.
• Documents patient visits in the electronic medical record per requirements.
