Job summary

UnitedHealth Group is seeking a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients' homes under physician orders. You will document visits, coordinate care, and work under supervision to ensure high-quality patient outcomes in your geographic service area.

The role emphasizes infection control, accurate charting, and collaboration with the health care team to support patients and families in a home setting.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws.
• Documents patient visits thoroughly in the EMR per regulatory and payer requirements.
• Participates in care coordination, communicates with patient/family/physician and case coordinators.
