Job summary

UnitedHealth Group is seeking a Home and Community Based Services Licensed Practical Nurse to deliver nursing care in patients\' homes. You will document visits, coordinate care, and follow care plans under supervision. CW nursing tasks include infection control and charting in the EMR.

Qualifications

Current and unrestricted Licensed Practical Nurse licensure in state of practice.
Current CPR certification.
Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
Ability to function in any home situation regardless of client characteristics.

Responsibilities

Provide direct nursing care in patients' homes as prescribed by the physician.
Record clinical and progress notes; coordinate other patient care activities.
Work under supervision of a Registered Nurse or designated supervisor.
