Job summary

UnitedHealth Group is seeking a Home Health Licensed Practical Nurse to provide direct nursing care in patients’ homes under physician orders and coordinate with the care team within the service area. You will document visits, monitor progress, and support infection control and regulatory compliance throughout care delivery.

The role emphasizes compassionate patient-centered care, strong communication, and collaboration with physicians, families, and case managers.

Qualifications

• Current CPR certification or ability to complete within 90 days of hire.
• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides high quality clinical nursing services within the scope of practice, as defined by state laws and plan of care.
• Documents patient visits thoroughly in the EMR per regulatory and payer requirements.
• Participates in care coordination, reporting needs and changes in condition, and communicates with patient, family, physician, and care team.
