Job summary

Monogram Health is seeking a Registered Nurse to serve as a Care Manager, delivering in-home care and care plan execution for patients with CKD/ESRD. You will collaborate with physicians, social workers, and care teams to improve quality of life at home.

You will assess patient needs, educate families, and coordinate with dialysis providers while traveling locally to patient homes. This role requires BLS and active RN licensure.

Qualifications

• RN degree and state licensure required
• 2+ years in care management or CKD/ESRD care
• Willingness to travel locally
• BLS certification required or will be supported

Responsibilities

• Develop and adapt care plans with patients and providers
• Conduct in-home care management visits
• Monitor biometric data and follow protocols
• Inventory and reconcile medications with pharmacists
• Perform patient health assessments and surveys
• Educate on CKD, ESRD, dialysis, and comorbidities
• Encourage adherence through regular contact with patients
• Engage family and social support groups in care
• Serve as primary contact during business hours
• Educate on medications, conditions, diet, exercise, and lifestyle
• Discuss Advance Care Plans and ESRD treatment options
• Record vitals and escalate concerns to the provider
• Facilitate onboarding and enrollment of patients
• Conduct post-op and discharge visits during transitions
• Document updates in the care management platform
• Coordinate with dialysis providers for seamless transitions
