Job summary

Monogram Health is seeking a Registered Nurse to join the Care Manager team. You will manage in-home care plans, monitor patient health, and collaborate with physicians and social workers to improve quality of life for CKD/ESRD patients.

Qualifications

• Frequent local travel to perform in-home visits.
• BLS certification required with onboarding support.
• RN license in the state of posted location.
• 2+ years' experience in care management and/or CKD/ESRD patients.
• Ability to take calls remotely on some nights and weekends.

Responsibilities

• Develop and continually adapt care plans with patients’ medical providers.
• Perform in-home care management visits to execute care plan.
• Monitor biometric data and follow protocols for interventions.
• Inventory and reconcile medications with pharmacists and prescribers.
• Conduct patient health assessments and required surveys.
• Educate patients on CKD, ESRD, dialysis and comorbidities.
• Encourage treatment adherence through regular contact with patients.
• Engage family and social support groups in education and care.
• Serve as primary contact during business hours for patient questions.
• Provide education on medications, conditions, diet, and lifestyle choices.
• Discuss proactive care decisions, including Advance Care Plans.
• Record vital signs and escalate concerns to providers.
• Onboard new patients and initiate relationships.
• Support transitions post-op and after hospital discharge.
• Document patient updates in care management system.
• Coordinate with dialysis providers for seamless transitions.
