Job summary

UnitedHealth Group seeks an RN for in-home hospice care in Texas. The role requires traveling within a 50-mile radius to assess patients, coordinate plans of care, and deliver comprehensive nursing services in the home setting.

Current RN licensure in Texas and 1+ years of clinical experience are required; travel and a valid driver's license are also necessary. CPR is preferred, with a strong emphasis on compassionate, independent patient care.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Make the initial nursing evaluation within 48 hours of referral to determine hospice eligibility.
• Identify and reassess patient/family needs every 15 days.
• Initiate and coordinate the plan of care.
• Document problems and patient/family responses to hospice care.
• Collaborate with IDG members in daily patient care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on patient status.
• Provide care irrespective of age, race, creed, or disability.
