Job summary

Bluebonnet Health Services is seeking an experienced Registered Nurse to administer nursing care in patients' homes and coordinate care with the interdisciplinary team, patients, families, and referring agencies. The RN will ensure high-quality hospice nursing care and adapt to diverse home environments.

The role requires an active RN license, valid driver's license, and at least two years of clinical nursing experience, with hospice experience preferred.

Qualifications

• Current state license as a Registered Nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact.
• Current driver's license.
• Two (2) years experience as a registered nurse in a clinical care setting.
• Hospice experience preferred.
• Must read, write and comprehend English.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs in the home setting.
• Applies and develops the plan of care to ensure quality and continuity of care.
• Education to patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
• Coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency.
• Provides on-call responsibilities and on-call services to patients and their families as assigned.

Education

Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
