Job summary

Venture I Inc. in Texas is seeking a compassionate Registered Nurse (RN) for Home Health & Hospice to deliver skilled nursing care in patients' homes and hospice settings.

The RN will collaborate with physicians, families, and an interdisciplinary team to ensure high-quality clinical outcomes and patient comfort. The ideal candidate will have an active Texas RN license, BSN or ADN, CPR (BLS), and at least one year of clinical nursing experience, preferably in home health or hospice.

Qualifications

• Active Texas RN license (or compact state).
• BSN or ADN from an accredited school.
• Current CPR (BLS) certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments including physical and psychosocial factors.
• Develop individualized care plans with physicians and families.
• Administer medications, IV therapies, wound care and other skilled nursing procedures.
• Monitor patient progress and report changes to the care team.
• Educate patients and families on disease management and home care.
• Coordinate care with interdisciplinary teams and document in EMR.
