Job summary

Hospice of South Texas (HOST) in Victoria, Texas, is seeking a compassionate PRN Registered Nurse (RN) to join our homecare clinical team. The RN will deliver skilled nursing care in patients' homes, coordinating with the interdisciplinary team to ensure compassionate, quality hospice care.

Ideal candidates have a Texas RN license, current driver’s license, and hospice experience. Flexible schedule, on-call duties, and the ability to travel locally are required; CPR certification is also

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor School of Nursing.
• Current Texas RN license and Texas Driver's License.
• Minimum two years RN experience in a clinical setting; Hospice experience preferred.

Responsibilities

• Perform comprehensive patient assessments to identify needs.
• Develop and update plans of care.
• Provide direct nursing care and symptom management.
• Coordinate services with physicians, aides, LVNs, social workers, chaplains, and team members.
• Educate patients and families regarding care needs and disease progression.
• Ensure accurate and timely clinical documentation.
• Maintain compliance with hospice regulations and payer requirements.
• Participate in on-call rotation as assigned.
• Promote HOST values and quality patient care.
• Other duties as assigned.
