Job summary

Compassus is seeking a Hospice Registered Nurse (RN) Case Manager in Texas to provide compassionate in-home hospice care. You will assess patient needs, develop and implement care plans, and coordinate with a multidisciplinary team to support patients and families at a challenging time.

Requirements include an active RN license in Texas, 2 years of nursing experience (hospice preferred), current CPR/BCLS, and a valid driver's license.

Qualifications

• Active RN license in Texas.
• Minimum of two years nursing experience (hospice preferred).
• Current CPR and BCLS certifications required.

Responsibilities

• Provide compassionate in-home hospice care and build trusting relationships with patients and families.
• Develop and implement individualized care plans in collaboration with the multidisciplinary team.
• Educate families and caregivers on patient care, comfort measures, and symptom management.
• Coordinate with physicians, social workers, and other team members to ensure quality end-of-life care.
