St. Luke’s is seeking a Hospice Case Manager to coordinate individualized in-home care for adult patients, collaborating with physicians and a multidisciplinary team to ensure the highest standard of care from admission to discharge in the Boise area.

This full-time role involves travel, rotating on-call, competitive hourly pay, bonuses where applicable, and reimbursement for mileage. We offer tuition and certification reimbursements and a supportive, inclusive culture.

Qualifications

• ASN or BSN in Nursing required.
• 2 years of RN experience; hospice case management preferred.
• Current Idaho RN license and BLS certification.
• Valid driver's license and auto insurance.

Responsibilities

• Manage Home Care caseload with travel within Boise and surrounding regions.
