Job summary

Idaho Home Health and Hospice of Treasure Valley is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients, collaborating with the care team to promote physical, psychosocial, and spiritual well-being.

Responsibilities include evaluating eligibility within 48 hours, ongoing reassessment every 15 days, initiating and coordinating the plan of care, documenting progress, and guiding patients and families in self-care to support quality of life.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current CPR Certification.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and re-assesses every 15 days.
• Initiates and coordinates the plan of care with the IDG.
• Documents problems, goals, interventions, and patient/family response.
• Collaborates with patient/family, attending physician, and team daily.
• Instructs patient/family in self-care techniques when appropriate.
