Job summary

St. Luke’s Health System in McCall, ID seeks a Home Health Case Manager to coordinate in-home care for a designated caseload, collaborating with physicians and a multidisciplinary team.

This direct-care role requires travel within McCall and surrounding regions from admission to discharge. You will support Home Health and Hospice patients, manage caseloads, and receive hourly pay with drive time and mileage reimbursement, plus on-call pay and relocation assistance where applicable.

Qualifications

• ASN or BSN in Nursing.
• 2 years of RN experience; Home Health or Acute Care preferred.
• Current RN license in Idaho; BLS certification.
• Valid driver’s license and proof of auto insurance.

Responsibilities

• Collaborate with physicians and multidisciplinary team to create and manage care plans.
• Deliver in-home care for Home Health and Hospice patients in the McCall area.
• Travel within McCall and surrounding regions from admission to discharge.
• Coordinate with care teams to ensure high-quality patient outcomes.
