Qualifications

• Associate degree in nursing required; bachelor’s preferred.
• Minimum 3 years RN experience in acute care.
• Current RN licensure and BLS certification required.

Responsibilities

• Provide in-home, acute patient-centered care as part of a collaborative team.
• Support Hospital at Home 24/7 with virtual assessments and monitoring.
• Assist in launching and growing the program; on-site provider available 24/7.
• Maintain RN/patient ratio 6:1.
