Job summary

Summit Home Care and Hospice is seeking a qualified Medical Social Worker (MSW) to join our Home Health and Hospice team in Columbus, OH. The MSW will assess psychosocial needs, develop care plans, and support patients and families through end-of-life care, bereavement, and community referrals.

Candidates must hold an MSW and Ohio LSW, have at least two years of healthcare social work experience, and possess excellent communication.

Qualifications

• MSW from an accredited school; must hold Ohio LSW license.
• Minimum of two years of social service experience in healthcare or social services.
• Knowledge of community resources and ability to obtain them for patients and families.
• Understanding of hospice philosophy and death/dying.
• Valid driver’s license and insured automobile in good working order.
• Excellent observation, judgment, and written communication skills.

Responsibilities

• Conduct initial and ongoing psychosocial assessments of patients and families/caregivers.
• Develop and implement care plans including social service goals and counseling.
• Provide pre-bereavement and bereavement support and groups as appropriate.
• Advocate for patients and families; connect to community resources.
• Assist with advanced directives, end-of-life planning, and facility placements when appropriate.
• Participate in discharge planning and IDG meetings every 15 days.
• Maintain accurate records and ensure compliance with laws and policies.
• Manage related expenditures and perform other duties as delegated.
