Job summary

Grace Hospice, a part of LHC Group, is seeking a Home and Community Based Services Aide to provide in-home hospice care and assist with daily living tasks, enabling patients to remain comfortably at home.

Responsibilities include personal care, household tasks, vitals monitoring, and documenting conditions. Candidates must hold Oklahoma Long Term Care Nurse Aide and Home Health Aide certifications, be able to travel within a 50-mile radius, and work flexible hours.

Qualifications

• Current Oklahoma Long Term Care Nurse Aide and Home Health Aide certification is required.
• 6+ months of home care experience.
• Ability to work flexible hours and independently.
• CPR certification or ability to complete within 90 days of hire (preferred)

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
