Job summary

Choice Health at Home is seeking a Hospice Aide to provide compassionate in-home care under RN direction. The role includes aiding patients in daily activities, supporting safe transfers, and reporting changes to ensure continuity of care.

Requirements include CNA/HHA and LTC certifications, CPR, driver's license, and auto insurance. HomeCare HomeBase experience is preferred. This full-time position travels weekly to Ada for supplies and serves patients in the Antlers, OK area.

Qualifications

• Current CNA/HHA & LTC certifications required.
• Valid Driver's License required.
• Valid Auto Insurance required.
• CPR Certification required.
• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care to patients under the RN supervision and according to the Plan of Care.
• Assist patients with safe transfers and ambulation.
• Document and report changes and care rendered to ensure continuity of care.
• Maintain infection control principles and provide a clean, safe environment.
• Communicate respectfully with patients, families, and staff.
• Respect patient privacy and property at all times.
• Assist with proper nutrition and adequate fluid intake.
• Support efficient operation and time management within the agency.
• Perform treatments and procedures per agency policy.
• Promote the agency philosophy and administrative policies to ensure quality of care.
