Job summary

UnitedHealth Group is seeking a Per diem CNA Hospice Aide to provide compassionate, hands-on end-of-life care to patients at home. You will support patients and families, follow the care plan, and work closely with the hospice team to maintain comfort and dignity.

Responsibilities include personal care, mobility assistance, meal support, and documentation. A valid CNA certification and CPR are required, with additional driving to patient homes as needed.

Qualifications

• Current state CNA certification.
• Current CPR certification.

Responsibilities

• Provides personal care services to patients.
• Assists with daily living activities and mobility.
• Plans and prepares nutritious meals; assists with feeding.
• Performs homemaking and other environmental tasks as assigned.
• Observes and reports changes in patient status to the nurse.
• Follow written care instructions from a nurse.
• Assist patients with prescribed exercises.
• Performs simple urine tests (sugar, acetone, albumin).
• Other duties as assigned.
