Job summary

Grace Hospice, part of the Optum family, provides home health and hospice services to support patients in the comfort of their own homes. We are hiring a Home and Community Based Services Aide to assist with personal care and daily living activities for clients within a 50-mile radius.

Full-time position with standard hours (Mon-Fri, 8:00 AM-4:30 PM) and eligibility for a comprehensive benefits package; travel is required for patient care.

Qualifications

• Current Oklahoma Long Term Care Nurse Aide and Home Health Aide certification is required.
• Current driver’s license, vehicle insurance, and reliable transportation or access to public transportation.
• Preferred Qualifications: CPR certification or ability to complete within 90 days of hire.
• 6+ months of home care experience.
• Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
