Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for the Conemaugh Regional Hospice‑Johnstown program. The role delivers comprehensive hospice care, develops and implements the Plan of Care, and collaborates with physicians and the care team to promote physical, psychosocial, and spiritual well‑being for patients and families.

The position requires current RN licensure, a valid driver's license, and the ability to operate across home settings, with a salary range reflecting

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care and documents progress and outcomes.
• Collaborates with patient/family, attending physician and IDG members to provide care.
• Instructs and supervises the patient/family in self-care techniques when appropriate.
