Qualifications

• Meets the training requirements of the State.
• Completed a minimum of 75 hours of classroom and supervised practical training; 16 classroom + 16 practical hours.
• At least 18 years of age.
• Ability to read and follow written instructions and document care.
• Understands hospice philosophy; comfortable providing care to the terminally ill.
• Satisfactory references from previous employers.
• Is self-directing with minimal supervision.
• Comfortable with death/dying.
• Provides a calm manner in patients' homes.
• Has empathy for the needs of the ill, injured, frail and impaired.

Responsibilities

• Perform personal care tasks and attend to patient requests promptly.
• Communicate with patients and families with empathy and confidentiality.
• Provide baths, oral hygiene, shampoos, and bed linen changes.
• Assist with dressing, undressing, and mobility as assigned.
• Plan and prepare nutritious meals and assist with feeding.
• Observe, assess, and report changes to the Case Manager.
