Job summary

Qualifications

• Meets state training requirements.
• Completed 75+ hours classroom and 16 hours practicum.
• Minimum age 18 years.

Responsibilities

• Provide personal care including baths, oral hygiene, and dressing.
• Assist with meals, mobility, and transfers under supervision.
• Observe and report patient condition to the Case Manager.
• Communicate with patients and families with confidentiality.
• Maintain patient comfort and skin integrity.
