Job summary

Beth Israel Lahey Health at Home seeks a compassionate Hospice Aide to provide in-patient and home-based support per the plan of care. You will take vital signs, assist with bathing, grooming, nutrition, and ADLs, while ensuring safety and infection control.

Responsibilities include documenting care in the EMR, reporting changes, and participating in weekend/holiday rotations with a strong team. An NH LNA license and CPR within 30 days are required; prior hospice experience is a plus.

Qualifications

• High School Diploma or equivalent.
• Licensed Nursing Assistant - NH state.
• Valid Driver's License.
• CPR certified or within 30 days of hire.
• One year experience as a Hospice Aide preferred.

Responsibilities

• Take and record vital signs and provide basic nursing assistant tasks as per the plan of care.
• Perform bed baths, dressing changes, oral hygiene, ADLs and mobility assistance.
• Document care in EMR and follow infection prevention policies.
• Report condition changes to appropriate personnel; participate in rotations and in-service programs.
