Job summary

Trinity Health is seeking a Full Time Home Health Aide (HHA) / State Tested Nurse Aide (STNA) to support hospice patients in the Columbus area. You will work day shift, follow the RN care plan, and provide compassionate in-home care.

The role requires a high school diploma or GED, Ohio STNA certification or approved training, and at least 1 year of relevant experience. You must have a valid driver's license and reliable transportation.

Qualifications

• High school diploma or GED completed.
• State-approved training with competency evaluation.
• At least 1 year of experience as an aide in home care, hospice, or facility.
• Current Ohio STNA certification or approved nurse aide training with competency.
• Meets agency hiring standards with references.
• Able to work independently while maintaining professional relationships.
• Willing to stay up-to-date with agency policies and training.
• Valid driver’s license and reliable transportation.
• Commitment to mission, ethics, and compliance.
• Dependable, compassionate, and comfortable in a home environment.

Responsibilities

• Follow the care plan created by the RN.
• Share any changes in the client’s condition right away.
• Help with personal care: bathing, grooming, dressing, mobility.
• Assist with light household tasks: meals, laundry, linens.
• Take and record vital signs when ordered.
• Support therapy-directed activities like range-of-motion exercises.
• Assist with self-medication under RN guidance.
• Help clients safely use home equipment like wheelchairs or side rails.
• Perform basic tasks such as changing ostomy or external catheter care.
• Document visits clearly and timely.
• Attend case conferences when appropriate.
• Maintain clear, respectful communication with clients and the care team.
