Ohio's Hospice is seeking Personal Care Specialists (STNA/CNA) to support hospice patients in Newark, OH and surrounding communities. This field-based role involves hands-on personal care, comfort, and connection with patients during a critical time in their lives.

You will follow each patient’s Plan of Care, document observations, and collaborate with the care team. Weekend and holiday rotations are required, and a valid Ohio driver’s license is essential.

Formación

• Provide compassionate, patient-centered care.
• Must be reliable and punctual.
• Ability to follow Plan of Care and document care.
• Strong communication with RN and care team.

Responsabilidades

• Review each patient Plan of Care before visits.
• Assist with activities of daily living.
• Maintain a clean, safe home environment.
• Obtain vital signs and report to RN.
• Document care in electronic medical records.
