Job summary

Affinity Care of Ohio is seeking a Hospice Aide to work under RN supervision, delivering personal care and comfort in patients' homes. You will report observations, assist with daily activities, and maintain a clean, safe living environment per the care plan.

The role emphasizes compassionate care for the terminally ill, proper documentation, and collaboration with the care team to ensure quality, continuity, and safety of patient care.

Qualifications

• Completion of state-approved CNA course or equivalent.
• NAC certification.
• CPR (Healthcare Provider) certification with hands-on demonstration.
• Food handlers permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and care for the terminally ill.
• 1 year hospice/home health experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care tasks per patient assignment and address requests promptly.
• Provide patient and family with respectful communication and confidentiality.
• Maintain patient safety and proper use of equipment.
• Assist with baths, hygiene, grooming, and changing bed linens.
• Assist in dressing, feeding, and mobility as assigned.
• Support self-administration of medications as allowed by state regulations.
• Assist with light housekeeping tasks and tending to laundry.
• Document care and observations accurately and report changes to RN Case Manager.
• Participate in quality improvement activities.
• Adhere to care procedures and professional conduct.
