Job summary

UEW Health is seeking a part-time Nurse Coordinator to provide in-home patient care and field-based clinical support. The role collaborates with Case Managers, physicians, and the interdisciplinary team to ensure continuity of care and high-quality outcomes.

The RN will perform supervisory visits where needed, support care coordination, and participate in discharge planning while maintaining flexibility for occasional evenings or weekends.

Qualifications

• Active, unrestricted RN license in the applicable state(s) of practice.
• Current CPR certification.
• Prior home health or similar clinical setting experience preferred.
• Demonstrated clinical assessment, critical thinking, and communication skills.
• Working knowledge of applicable home health regulations, standards, and best practices.
• Ability to work independently while exercising sound clinical judgment in a field-based environment.
• Ability to meet credentialing and onboarding requirements, including background checks.

Responsibilities

• Perform in-home patient visits, including health and psychosocial assessments.
• Monitor patient condition and report changes to the Case Manager/physician.
• Provide patient and caregiver education on disease processes, treatments, and care plans.
• Complete and submit all clinical documentation accurately and timely.
• Provide coverage visits when an RN is unavailable to ensure continuity of care.
