Job summary

Qualifications

• Current RN license in good standing.
• At least one year of home health experience or related area with strong assessment skills.
• Excellent organizational and time management skills to manage multiple clients.
• Proficient with computer and internet-based applications and office equipment.
• Competent working knowledge of client-based equipment.
• Must pass criminal background check and sanction screening.
• Professional appearance and demeanor are required.

Responsibilities

• Manage nursing care, home health aide services, and therapies once approved by the Department of Labor.
• Maintain confidentiality of all client and office records in accordance with HIPAA guidelines and the Privacy Act PHI.
• Facilitate effective communication and collaboration among the care team to provide cohesive and comprehensive care.
• Perform other duties as assigned to support the team and enhance patient care.
