Job summary

Beth Israel Lahey Health is seeking a registered nurse to plan, organize, and direct home care services with emphasis on community health education. Care is delivered in patients' homes and communities in accordance with home care regulations and payer guidelines.

You will complete initial assessments, initiate and revise care plans as patient status changes, administer medications, and supervise home health aides every two weeks.

Qualifications

• Active Massachusetts RN license
• BLS certification active
• 1 year direct care nursing experience
• OASIS and/or EPIC experience preferred

Responsibilities

• Complete initial assessment of patient and family to determine home care needs
• Initiate and revise plan of care as patient status changes
• Administer medications and treatments as prescribed by physician
• Instruct, supervise, and evaluate home health aide care every two weeks
• Identify discharge planning needs as part of care plan development
• Maintain HIPAA confidentiality and agency policies
• Complete OASIS and EPIC documentation timely
