Beth Israel Lahey Health is seeking an experienced Home Health Registered Nurse to join our growing team. You will plan, direct, and coordinate home care services to meet patient and family needs in the home and community.

The role emphasizes community health education, OASIS completion, and EPIC documentation. Candidates must hold MA and NH RN licenses, a BLS certification, and a BSN or Associate’s degree.

Qualifikationen

• Experience in home care or public health preferred.
• Proficiency with initial patient assessments and care planning.

Aufgaben

• Plan, organize and direct home care services for patients and families.
• Complete initial patient and family assessments to determine needs.
• Initiate and revise the plan of care as patient status changes.
• Administer medications and treatments as prescribed by the physician.
• Instruct, supervise, and evaluate home health aides every two weeks.
• Identify discharge planning needs as part of the care plan development.
• Maintain confidentiality per agency and HIPAA guidelines.
• Complete OASIS and EPIC documentation in a timely manner.
