Job summary

Beth Israel Lahey Health seeks a registered nurse to plan, organize, and direct home care services within patients’ homes and communities. The role emphasizes community health education and coordinating care with families and providers.

The ideal candidate holds a Massachusetts RN license, BLS, and at least 1 year of direct care experience; OASIS/EPIC experience is preferred. A competitive salary and benefits package accompany the role.

Qualifications

• Active Massachusetts RN license.
• BLS certification from American Heart Association.
• 1 year of direct care nursing experience, preferably in home care or public health.
• OASIS and/or EPIC experience preferred.

Responsibilities

• Complete and document an initial patient/family assessment to determine home care needs.
• Initiate and revise the plan of care as patient status changes.
• Administer medications and treatments per physician orders.
• Instruct and supervise home health aides every two weeks.
• Identify discharge planning needs as part of care plan development.
