Beth Israel Lahey Health in Massachusetts is seeking a Registered Nurse to lead home care services, coordinating care for patients and families in their homes and communities. The RN uses community health education to plan and direct services in line with home care regulations and payer guidelines.

You will complete initial assessments, start and adjust plans of care, administer medications, supervise home health aides, and ensure timely OASIS and EPIC documentation.

Formación

• Active Massachusetts and New Hampshire RN licenses
• Active BLS certification
• Associate’s Degree or BSN (preferred)
• At least 1 year direct care nursing experience; OASIS/EPIC experience preferred

Responsabilidades

• Complete initial patient and family assessment
• Initiate and revise care plans as patient status changes
• Administer medications and treatments as prescribed by the physician
• Instruct, supervise, and evaluate home health aide care every two weeks
• Identify discharge planning needs as part of the care plan development
• Practice confidentiality per agency and HIPAA guidelines
• Demonstrate proficiency in OASIS completion
• Complete EPIC documentation timely
