Beth Israel Lahey Health is seeking a Registered Nurse to plan, organize, and direct home care services with emphasis on community health education. You will build from community resources to design services meeting patient and family needs in homes and communities, compliant with home care regulations and payer guidelines.

The role includes initial assessments, care planning, medication administration, and supervision of home health aides.

Qualifications

• Active Massachusetts RN license.
• Active American Heart Association BLS certification.
• Associate’s Degree or BSN from an accredited nursing program.
• At least 1 year of direct care nursing experience; OASIS and/or EPIC experience preferred.

Responsabilités

• Complete and document an initial patient and family assessment to determine home care needs.
• Initiate and revise the plan of care as patient status changes.
• Administer medications and treatments as prescribed.
• Instruct, supervise, and evaluate home health aide care every two weeks.
• Identify discharge planning needs as part of care planning.
