Beth Israel Lahey Health is seeking a licensed RN to lead home health services within the community. The role emphasizes planning, organizing and directing care in patients' homes, with a focus on public health education and compliant care delivery.

Requirements include an active MA RN license, BLS certification, and at least 1 year of direct care nursing experience; OASIS/EPIC experience is preferred. Competitive salary and generous benefits offered.

Competenze

Active Massachusetts RN license.
American Heart Association BLS certification.
Associate’s Degree or BSN (preferred) from an accredited school of nursing.
At least 1 year of direct care nursing experience; OASIS/EPIC experience preferred.

Mansioni

Complete and document an initial assessment of patient and family to determine home care needs.
Initiates the plan of care and revisions as patient status and needs change.
Administers medications and treatments as prescribed by the physician.
Instruct, supervise, and evaluate the home health aide care every two weeks.
Identify discharge planning needs as part of the care plan development.
Practice confidentiality per HIPAA guidelines.
Demonstrate proficiency in OASIS completion.
Complete documentation through EPIC per agency policies.
