Job summary

Beth Israel Lahey Health is seeking a dedicated Registered Nurse to plan, organize, and direct home care services in communities and patients’ homes. The role emphasizes community health education and adherence to home care regulations and payer guidelines.

Responsibilities include initiating and revising care plans, administering medications, supervising home health aides, coordinating discharge planning, ensuring HIPAA compliance, and completing OASIS and EPIC documentation to support

Qualifications

• Active Massachusetts and New Hampshire RN licenses (or willing to obtain the other within 3 months).
• Active BLS certification.
• Associate’s Degree or BSN from an accredited school of nursing (BSN preferred).
• At least 1 year of direct care nursing experience; home health/public health preferred.

Responsibilities

• Plan, organize, and direct home care services for patients and families.
• Initiate and revise the plan of care as patient needs change.
• Administer medications and treatments as prescribed by the physician.
• Instruct, supervise, and evaluate the home health aide every two weeks.
• Identify discharge planning needs as part of the care plan development.
• Maintain confidentiality per HIPAA and agency policies.
• Complete OASIS documentation.
• Document in EPIC per timeliness policies.
