Job summary

Signify Health, part of CVS Health, seeks a Full Time Physician to provide in-home health evaluations, engaging with members in their homes and bridging gaps in care.

In this role you will visit patients up to 40 hours per week, conduct medication and medical history reviews, physical evaluations, and optional diagnostic tests. Travel locally and within licensed states is required; virtual visits may be conducted as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi‑state licenses (preferred).
